What pressure will QEV's fail at?

So we've heard the stories of people using HPA and unregulated CO2 with their QEVs, reaching 800 psi, but has anybody ever had a QEV fail on them? What size was it, and at what pressure did something go wrong at?
I just ordered a McMaster 3/4" QEV that I plan to use at 500 psi. Reading through all the QEV related posts, it seems like I should be good at that pressure.
Pretty much every QEV article I could find, people have been using their valve reliably at whatever pressure they use, be it the rated pressure, 300psi, 500psi, or even higher.
How far are these pressure from the point at which the QEV is being severely damaged?
